Oak Creek Assisted Living and Memory Care

From the paint on the walls to the pattern of the floors, every detail in our Azura Memory Care of Oak Creek homes were purposely designed for those with early to late stage dementia and Alzheimer’s disease. With an emphasis on providing exceptional memory care, Azura of Oak Creek maintains a highly-skilled team trained specifically in dementia care in accordance with the Alzheimer’s Association’s Guidelines for Dignity and Azura’s exclusive MOSAIC training. This level of expertise and training is unique and not something you will see offered in other Oak Creek assisted living facilities and nursing homes.

Our MOSAIC education and engagement system trains our team on how to properly approach, communicate and engage those with memory loss. In addition, our team learns our resident’s personal preferences and histories and weaves those pieces into their care and engagement time. You will have peace of mind knowing your loved one is receiving high-quality memory care led by a team of dedicated directors, nurses, caregivers and therapists.

Our Oak Creek care campus features two homes located in close proximity featuring Azura’s MOSAIC Purposeful Destinations, multi-sensory rooms, a sound masking system, library, secured courtyard with walking path, and screened-in porches.  Each resident has their own furnished Studio Suite to rest their head, which features a private age-in-place bathroom with roll-in shower, built-in cabinetry, RCare response system, cable and phone hook-ups, and the encouraged option to personalize it with pictures and mementos of home. We also offer a Couples or Enhanced Suite for those looking for more space.

Personalized Suites:

“Thank you…from the bottom of my heart for accepting (name removed) into your care when many other facilities did not. I do not know what I would have done had you not. All of the employees, primarily at Studer house but also at Honadel are priceless. They are kind, loving, considerate, compassionate and respectful and do all day in and day out when I know it is not an easy job to do. What they all do requires a special type of person and you are lucky to have so many! –Judy, Ben, Greg

Azura Memory Care Oak Creek, Wisconsin FAQs

What is the difference between assisted living and memory care?

Assisted living and memory care offer distinct experiences for older adults. Assisted living focuses on elevating independence by providing support for daily personal care tasks, such as bathing, dressing, eating, and transferring.

In contrast, memory care involves 24/7 specialized support with a higher caregiver-to-resident ratio. While this care includes assisted living support, it also involves a dementia-friendly home, specially trained staff, and tailored activities to minimize cognitive decline.

Does Medicare pay for dementia care?

Typically, Medicare covers any medically-necessary care, including doctor’s appointments, treatments, and therapies. However, it doesn’t extend to the general living costs at memory care, such as meals, housing, personal care support, and activity programming.

What are the signs that it’s time for memory care?

Individuals develop dementia symptoms under unique timelines. Yet, it may be time for memory care if your loved one is showing a decline in overall health, as well as symptoms such as heightened anxiety, confusion, and aggression. This is especially true if they’re prone to falls/injuries, wandering, or socially inappropriate behaviors.

In addition, many families who feel burnout from caregiving hope to restore their relationship with their parents by seeking outside help.
