Jake shares, “With years of experience in caring for individuals with dementia, I am dedicated to making a positive impact in the lives of those affected by Dementia. Currently serving as a MOSAIC & Engagement Specialist at Azura of Brookfield, I bring a wealth of hand-on knowledge and a compassionate approach to my work.
My journey with Azura began three years ago. I’ve worked at 3 of 14 locations, each in a different capacity. These experiences have provided me with invaluable insights into the unique challenges and needs of individuals with Dementia.
To further enhance my ability to provide comprehensive care, I hold certifications as a Certified Dementia Practitioner, CBRF Administrator, and Certified Nursing Assistant. These credentials, along with my extensive hands-on experience, enable me to offer exceptional support and knowledge of care to our team members and the community.
Recognized as Azura’s True Blue Award Winner of 2021, I pride myself on my commitment to ongoing learning and professional growth. I consistently seek opportunities to expand my knowledge and expertise in the field of Dementia care, ensuring that I stay up to date with the latest advancements and best practices.
In the Brookfield region, I play a pivotal role in facilitating Azura’s T.I.M.E. Program, an early memory loss initiative that brings joy, enrichment, and personalized care to individuals in the community. Additionally, I lead our MOSAIC trainings which are designed to empower caregivers with the knowledge and tools they need to create a compassionate and engaging environment for people with Dementia.
Committed to making a difference beyond the walls of our community, I actively contribute to the Brookfield area by facilitating a caregiver support group and memory cafe. Through these gatherings, I provide a nurturing space for individuals caring for their loved ones with memory loss to connect, share experiences, and find solace in a supportive community.
Driven by my belief in the power of education, I thoroughly enjoy providing valuable insights, practical tips, and effective strategies to those caring for a loved one with memory loss. As an advocate for compassion and understanding in dementia care, I am passionate about helping others see situations from a different perspective, fostering a more empathetic and inclusive approach.”
