
Ms. Kreider manages the payrolls and accounts payable for Azura Living
and Azura of Lakewood – Rehabilitation Suites. Jill developed and
implemented accounting practices and payroll services, and is an
instrumental team member of the Azura Leadership Team which serves to
guide the growth of the organization.
Ms. Kreider worked in the ski
industry as an Accountant for Intrawest at the corporate headquarters in
Golden, Colorado. Her role required her to work closely with ski
resorts all over the U.S. to simplify processes and ensure compliance.
She also implemented projects to improve accounting processes while
saving significant money.
Growing up in the small town of Estes Park
and working through college, Jill has a diverse background in the
hospitality/restaurant industry and sees Azura as a great opportunity to
apply her knowledge, transform the culture of care and redefine senior
care and focus on healing the whole entire person.
Jill has a B.S.
from the University of Colorado in Boulder with an emphasis in
Accounting. She is currently working on an M.B.A. from the University
of Colorado in Denver. Ms. Kreider and her husband John currently live
in Lakewood, Colorado.

As our Director of Nursing, Laura keeps herself busy making sure guests
are well cared for and their needs are met. An integral part of our
care team, Laura is a certified CNA. A graduate from Front Range
Community and Community College of Denver, Laura is also about to
complete her BSN at University of Phoenix.
Laura is originally from
South Dakota and grew up in Colorado. She has three children and two
Labrador retrievers and is a crime thriller junkie – CSI Miami and NCIS
are two particular favorites.

Jonna brings a smile and sense of humor to Azura. She is driven by a
passion to contribute to society and believes that every person deserves
to be well cared for and treated with dignity and respect. Originally
from Kentucky, Jonna went to Eastern Kentucky University where she
received a bachelor degree in Occupational Therapy. Jonna has spent
over twenty years specializing in geriatric care as well as home health
care. Formerly with Lifecare, Jonna's experience helps her to
anticipate and understand the needs of patients and their families.
Jonna is married and has two wonderful children. She is also involved in Whiz Kids, a tutoring program for at-risk children.

Debbie holds a Bachelor’s degree in Science and Nursing from the
University of Colorado. In the early part of her career, Debbie was a
PICC line certified IV nurse. She has worked as Porter and Children’s
Hospitals. Apria and Lifecare and has been a liaison for over ten years.
A consummate professional, honesty and integrity are important values
to Debbie, who is proud to represent Azura.
Debbie is married with two adult children. She enjoys the outdoors, working out, dance and gardening.

Dina holds a Bachelor’s degree from Colorado State University and a
Master's degree from University of Northern Colorado where she was
trained as a Speech Language Pathologist. Over the past thirteen years
Dina has served in various marketing and admissions roles within the
senior care industry. Dina enjoys her current role as Director of
Admissions as it allows her the opportunity to interact with guests and
serve an important role in ensuring that their first experience at Azura
of Lakewood is positive and friendly.
Dina is married and has a beautiful son. Being a mother is the role she is most proud of.

Freddy comes to Azura with a great degree of customer service
experience. He served as a Concierge Supervisor for JW Marriott
Quito-Ecaudor and was nominated for the J. Willard Marriott Award of
Excellence. Freddy has several certifications in leadership and
customer service as well as an Environmental Health Certification
through the Colorado Health Care Association. He graduated with a
degree in management from Bucks County Community College. Freddy's
vision at Azura is to set the highest standards for environmental
services across the region to ensure that every housekeeper is trained
and empowered to be exceptional and make a difference in our guest's
lives every day.
Freddy was born and raised in Ecuador. He is a Spanish tutor, enjoys puzzles and plays soccer in his spare time.

As Director of Plant Operations, Ray makes sure the building is running
smoothly and efficiently. Prior to joining Azura, Ray worked at Pinon
and the Ensign Group. Ray has been on the Workman's Compensation Safety
Committee since 2009, is a member of the Colorado Healthcare
Association and is on their OSHA Implementation Team. He attended Palm
Beach Community College and the University of Miami and has an
Associate’s degree in business.
Ray moved to Colorado 13 years ago and enjoys hiking, backpacking and skiing. He has been married 27 years and has a daughter.

Audrey brings a new tune to Azura. A Board Certified Music Therapist,
she also has a Masters of Music and is a Neurologic Music Therapist
Fellow. As Director of Life Enrichment and Wellness for Azura, she
strives to collaborate daily with the therapy department, to make the
experience beneficial and meaningful for all guests. She is a graduate
of University of Nebraska at Omaha and Colorado State University. She
most recently served as the Director of Restorative Therapy and Music
Therapist at Julia Temple Healthcare Center.
Audrey sings for the
Littleton Chorale, loves spending time with her toddler, surrounding her
day with music and making people smile.

Leslie attended Regis University and is a registered Health Information
Administrator (RHIA). At Azura, she works hard to achieve successful
implementation of the electronic health record while touching the hearts
of guests and their families. Her previous roles include Director of
Health Information Management at Colorado State Veterans Home Fitzsimons
and Highline Rehabilitation and Care Community. She also serves as
Mom, spiritual leader, mentor, educator, mental health care provider and
friend.
Leslie admires her four children - Josh, Yoni, Doron and
Aliza. They are all magnificent people and taught me what it is to live
life to the fullest.

Katy has a BFA from Metropolitan State College of Denver. As the
billing and collections specialist at Azura, Katy is consistently
defining and improving Azura's billing and collections policies and
procedures. She has worked in SNF/LTC billing for six years in Skilled
Nursing and Long Term Care and has learned everything she knows on the
job.
Katy enjoys running, cycling, sunshine,printmaking, sewing, and food grown by her husband.

Certified Executive Chef Greg Sever received his training through the
American Culinary Federation Apprenticeship Program. This is a
comprehensive, European-style apprenticeship which includes
certification in management, sanitation, and nutrition.
Dedicated
with the company’s mission to Transform the Culture of Care, Greg and
his staff are on a mission daily to bring the most wholesome, nutritious
and balanced meals possible to guests at Azura of Lakewood. Greg feels
that giving back is important and does this by mentoring supporting
future chefs. He was recently elected President of the Rocky Mountain
Chefs of Colorado, which he considers a proud accomplishment.
When
he's not at work, Greg spends his time with his wife and three children
who enjoy the outdoors and are active in cello, basketball, and skiing.

Dr. Sheldon Goldberg is a Board Certified physician in physical medicine
and rehabilitation with a wide range of training and experience who
currently serves as Chief Medical Officer of the Azura Living
Corporation.
His medical and administrative background prepared him
for the success he has had as: attending physician, medical director,
chief medical officer and consulting physician in all areas of physical
rehabilitation such as survivors of multiple trauma, joint replacement
surgery, stroke, chronic wounds, and complicated hospitalizations.
Dr. Goldberg holds two Bachelor's degrees; one in dietetics and
nutrition from the Down State Medical Center in New York City, and the
second in health science from the University of the State of New York,
along with national certification as a physician's assistant. Dr.
Goldberg began a long career in health care in metropolitan Denver first
as a physician's assistant in the area of orthopedic surgery, arthritis
and rheumatic diseases and internal medicine. He then returned to
medical school. While receiving his MD degree from one of the major
universities in Mexico, he did his clinical training at Creighton
University and the University of Colorado and affiliated hospitals.
After two years of surgical training, he completed the residency in
physical medicine and rehabilitation at the University of Colorado
Health Sciences Center with the honor as serving as chief resident in
rehabilitation medicine and president of the 700-member House Staff
Association.
Dr. Goldberg has served as both a hospital based and
skilled nursing facility based medical director for most of his career.
He has been recognized by his peers in Denver's 5280 Magazine and Top Doctors in America
multiple times. Dr. Goldberg has done clinical research in the areas
of wound healing and pain management and has co-authored articles in
peer reviewed medical journals in the areas as of geriatric
rehabilitation and stroke prevention. Dr. Goldberg is an assistant
clinical profession in the department of physical medicine and
rehabilitation at the University of Colorado at Denver Medical School
and also served on the clinical faculty at the University of Nebraska
Medical School for more than 10 years.

Ms. Griffin has been in a professional in the Human Resources field for fourteen years. She has supported two Fortune 500 Companies and has been supporting healthcare organizations for the past several years. Ms. Griffin has a very strong background in Organizational Development, particularly around building a positive culture and helping organizations become an Employer of Choice. As the Manager of the COBE Development Center, Ms. Griffin conducted needs analysis surveys and created and delivered customized training for the organization. She also counseled employees at every level of the organization on development plans that would enhance their career opportunities. As the Manager of the Coors/Regis Partnership, Ms. Griffin continued to develop and deliver customized training for every department and was an integral member of the Organizational Development team. At Coors, Ms. Griffin continued to work with individuals who wished to develop themselves in their career paths. She also supported organization wide change management and Employer of Choice initiatives and became an expert on employee engagement when her team consulted with key executives from the Gallup Organization. Ms. Griffin became a well-rounded Human Resources professional as a Generalist for AutoNation and later as a Sr. Human Resources Representative for a local non-profit healthcare organization. Prior to Ms. Griffin’s work as a Human Resources professional, she was a paralegal and worked in the municipal and state court systems. Her legal background enhances her work in Human Resources as she manages employee relations and creates policies that protect the company and its employees. Ms. Griffin earned her B.S. from Colorado State University and went on to earn a Paralegal Certificate from Denver Paralegal Institute. After working as a Paralegal for several years, she went on to earn her M.A. in Counseling and Human Resources from the University of Colorado at Denver in 1997. Since then, Ms. Griffin has earned her Senior Professional in Human Resources Certification (SPHR) and has also earned affiliation with the National Board of Certified Counselors (NCC). Ms. Griffin and her husband, Greg, reside in Thornton, Colorado. They have a son and daughter in law, Mitch and Angela, who also reside in Thornton and a daughter, Angie, who resides in Denver, Colorado.

David is the newest addition to Azura’s HR team as the Human Resources Professional. David has over 15-years of combined HR experience in staff development, diversity, recruiting and human resource management. After a distinguished career in the military that included experience as a flight medic, Paramedic Instructor, Staff Development Coordinator, Recruiter, and Human Resources Manager, David worked mainly as an HR consultant while pursuing his degree. Using his understanding of strategic, proactive processes, David helped several companies significantly improve their HR practices, recruitment and retention, and strategic training needs. David is a certified Instructor-Trainer in targeted selection and Behavioral Interviewing by Development Dimensions International (DDI). David earned a B.S. in Human Resource Management from Regis University in Denver, CO and is currently pursuing his graduate degree in Behavioral Science.
