Care Transitions Coordination is standard practice here at Azura Living and once you leave our Home we continue to provide follow-up care at your Home to ensure enhanced quality of life and independence through our affiliate Azura At Home.

Skilled home health services are generally covered by most medical insurance. Medicare covers home health care at 100%. Azura at Home accepts Medicare, Medicare advantage insurances, several Major Medical Insurances, and private payment.

The staff at Azura at Home understands the regulatory burdens of making a home care referral. We are here to help in every way possible. We are constantly working on streamlining the referral process and can accept referrals by fax, phone, and electronically.
If desired, we can have a representative come and collect the information and meet the patient prior to a procedure or discharge from a facility.
Azura at Home realizes the importance of good communication and will keep the primary physician informed of a patients’ status. We work with a number of facilities and physician practices in order to follow specific protocols and treatment regimes.
